- If your child’s doctor has referred your child for an Applied Behavior Analysis (ABA) assessment, you will need to provide us with a referral from the doctor. You will also need to submit a Comprehensive Diagnostic Evaluation (CDE).
- Call your health insurance company to ensure that they cover Applied Behavior Analysis or Behavior Analysis services. It is also important to inquire about any deductible or copay you might have.
- Print and fill out the Better Life Behavioral Services patient information and consent forms.
- Once you have a referral, CDE, insurance coverage, and you complete the patient information packet, please email it to info@betterlifeofcentralfl.com , fax, or drop it off at the office.
- After the referral, CDE and patient information is received you will have to call to schedule an appointment to meet with a BCBA for an initial assessment and during that appointment the following will occur:
- Parent/Caregiver and Client Interview: Collect detailed information about the individual’s developmental history, medical background, family dynamics, and specific concerns. This often involves structured interviews and questionnaires.
- Review of Records: Examine existing medical, educational, and therapeutic records to gain a comprehensive understanding of the individual’s history and prior interventions.
